GENERAL WCA ATHLETIC INFORMATION:

Please make sure that your athlete has a current Athletic Medical Form on file with the Athletic Department which will be valid for the duration of the season in which your child is participating.  This form must be signed and dated by a physician.

Athletic Medical forms are valid for 12 months.  Blank forms can be found under Resource Documents on RenWeb and on our Google athletic website (link is posted above).  Many CVS locations have Minute Clinics where a physical can be performed with no appointment required.  For more information please visit www.minuteclinic.com.  An athlete cannot begin the sports season until this form is completed, signed and dated by a physician, and filed in the athletic department.

Coaches will be given a medical emergency form for each athlete on their team based on the information you have given to the school in your RenWeb account.  Coaches will be given a medical emergency form for each athlete on their team based on the information you have given to the school in your RenWeb account.  These forms are printed out of RenWeb and contain the same information a teacher would have if your child went on a field trip.   To edit your child's medical information in RenWeb, use the "Family Demographic Form" under "Web Forms."

Any uniforms not turned in at the end of the season will result in a fine.  At the end of each season, we will have a ceremony/dessert for athletes and their families. Awards will be given out and team pictures will be taken.  After pictures are taken, uniforms will be turned in to the coach for the season. If for some reason your child misses this event, they have one week following that date to return their uniform before the fee is assessed.

WE NEED DRIVERS

For some games, the activity bus and the yellow bus will not be available and parent carpools will be necessary. Please let your coach know if an adult in your family is willing to be a driver when possible. Students may not drive other students.
